The Clinical and Social Work Services (CSWS) Department at St. Ann's Center provides trauma-informed, strengths-based, and individualized services to support pregnant and parenting young women as they work toward emotional well-being, stability, and independence.

Clinical & Social Work Services

Upon admission, each resident is assigned a Case Manager or Social Worker who works collaboratively with her to identify individual needs, establish goals, and develop an individualized service plan. Through ongoing case management and team meetings, residents receive support, guidance, and advocacy as they work toward achieving their goals.

Our department provides a range of clinical and supportive services, including assessments, individual therapy, therapeutic groups, crisis intervention, and referrals for additional mental health services when appropriate. We recognize the importance of creating a safe and supportive environment where residents feel heard, respected, and empowered.

CSWS also coordinates the initial screening and admissions process for prospective residents, helping to determine eligibility and assess whether our programs can meet their individual needs.

Our team works closely with families, child welfare agencies, healthcare providers, and community partners to coordinate services, address barriers, and advocate for residents' needs.

As residents prepare to transition from St. Ann's, our team assists with discharge planning, connections to community resources, and aftercare support when applicable.

Our goal is to help each resident recognize her strengths, navigate challenges, strengthen relationships, and build a foundation for continued growth and stability.
